Records Request

Immunization Records Request

The Student Health Service retains medical records for 7 years after you have graduated/moved on from SUNY New Paltz. If you would like your immunization records sent to a new school, doctor, employer etc. please complete and submit an Immunization Records Request Form. If you would like the records sent to you personally, please write your name and information where it asks for 'Facility'.

This form may be submitted to us via any of the following methods:

Once this form is received by the Student Health Service, it will take 48-72 hours for us to process your request.

Medical Records Request

If you would like any medical records (aside from your immunization records) sent to another facility, please complete and submit a Release of Records From Student Health Service Form.

This form may be submitted to us via any of the following methods:

Once this form is received by the Student Health Service, it will take 48-72 hours for us to process your request.

**Please note, the Student Health Service is not permitted to share any well-physical information. Well-physicals are 3rd Party Records that belong to the provider who completed the physical. You will need to contact your primary doctor directly if you would like a copy of your physical.

If you need any medical records sent from another facility (doctor, school, hospital, specialist etc.) to the Student Health Service, please complete a Release of Records To Student Health Service Form and submit it to the facility that will be sending the records to the Student Health Service. If you need the Student Health Service to submit the request on your behalf, submit the request to us and we will forward it to the appropriate party. You may call us at 845-257-3400 to ensure your records have been received.
