Please complete the Name/Address change form to update your record with Human Resources, Benefits and Payroll. Forward completed hardcopy of form to Payroll Department (HAB 301).
For name change only please include a copy of Social Security card with your new name, and complete new tax forms: W-4 and IT-2104.*
