Based on information in the records and registration system we will produce a packet for each course and section as it is recorded in the system.

Three weeks before final exams packets for the following schools can be picked up from these locations:

-Business - in the dean's office
-Education - in the mailroom in SCB, Room 129
-Fine and Performing Arts (except Music)- the dean's office
-Fine and Performing Arts - Music  - College Hall 100

-Honors Program - Director's Office - College Hall H-111
-Interdisciplinary courses - in the department or dean's office that faculty members are from
-Liberal Arts and Sciences - individual departments
-Science and Engineering - in the dean's office

Packets will be in crates/boxes, organized alphabetically by faculty last name. They will be in a secure location (minimizing the accessibility to students for example).

Verifying your packet at pick-up

It is important that ANY questions you have at the time you pick up the packet that you contact Institutional Research directly, at ext. 3228, not a dean or department secretary.

It is critical that you verify the accuracy of the information on the front of the packet by using the mini-checklist on the envelope.

Each course is assigned a CRN number, therefore, it is important that you use the packet that matches the course you want evaluated. CRN's are used in the scanning process. If there is a mix up with the packets, please contact Institutional Research at x.3228 so the problem can be corrected.

Sign the front of the envelope.

There will be a binder located near the SEI bin to sign your packet out.

Pick up pencils available at the SEI box (Please take only what you need).

To ensure anonymity, courses with enrollment of less than three will not be processed (exception - see combining courses).
