HOW TO APPLY FOR A TRAVEL SIGNATURE ON THE I-20 OR DS-2019
Notice:
If you plan to travel outside the US, you might need a travel signature on the I-20 page 3 / DS 2019 page 1. If the last signature will be more than 6 months old when you re-enter the US, you should get a new travel signature.
- Fill in the TRAVEL SIGNATURE FORM *
- Hand in the form with copies of the following documents to the Center for International Programs:
Passport information pageVisa in passport
I-94 card (both sides)-white card stapled inside passport - Hand in original I-20/ DS-2019
- Please come back in 3 business days to get your I-20/ DS-2019
