Students must submit a satisfactorily completed Pre-Enrollment Health Report and Physician's Certificate. For questions about the proper completion of health reports that students, their families and their personal physicians feel are not covered by the written materials included on the Health Report and in the College catalogue, concerned individuals are encouraged to contact the Student Health Center for further information.
Students who have not submitted a completed Health Form will not be eligible for treatment in the Student Health Center but may contact the Center for professional advice in securing appropriate medical care.
