Overseas travel is an opportunity for students to experience different cultures. Different cultures sometimes present different health concerns. Preparing for overseas travel is also an opportunity to make sure routine vaccinations have been given and a recent health exam has been performed.
If you would like Student Health Services to perform and document your OSPE then you need to:
1. Make an appointment at the Health Center.
2. Prior to that appointment you must download the OSPE form below and complete the Student Health Information page and the top portion of the Physician's Statement page.
3. These completed forms must accompany you to your scheduled appointment at the Health Center.
4. If other forms are necessary for travel, they should be completed and brought to your scheduled appointment.
5. Additional time is required for OSPE appointments. Therefore be on time and bring all appropriate documents.
