Meningococcal Vaccination:
While the meningococcal vaccine is not required, it is strongly recommended. Pursuant to NYS law every student registered for six credits or more must submit to the Student Health Center a properly completed, signed Information Response Form. These forms are sent to all applicants. The Form may be downloaded below. A properly completed Form indicates that the student has read the information on meningococcal disease and the vaccination, and indicates whether the student has received or refused the vaccine. Under the current law, failure to return this form within 30 days of the beginning of the student's first semester could impact the students' registration status.
