To apply for graduate studies using a paper application, you may either request an application packet from our Graduate Admissions Advising office or download the application from our website. There are three different paper applications:
- Graduate School Application—used by domestic applicants and permanent residents of the US
- International Graduate Student Application—used by international applicants (includes I-20 and visa information)
- MBA Application—used by domestic and international applicants for the MBA program only.
Applicants choosing to submit their application via our traditional paper application booklet will still be able to check their application status online. Once a complete application packet is received and processed, the applicant will receive login information to check the status of his/her application online by mail or e-mail.
Paper applications must be submitted if the applicant:
- prefers to pay the application fee by check or money order
- is applying after May 15 or Nov 15 for a program with a *rolling admission deadline
Paper applications will be accepted according to the following schedule:

CAS in Clinical Nurse Specialist: Adult Health (Major Code 400)
